This privacy notice (“Privacy Notice”) explains how Curatek Pharmaceuticals of 1965 Pratt Blvd., Elk Grove Village, IL 60007 (“Curatek”) handles personal information given to us on websites, mobile sites, and other services and products that link to this Privacy Notice. Please read this information carefully.

The types of personal information that we may collect and receive are described here and include information that you give us and information that we collect automatically when you use Curatek’s services.

“Personal Information” means information that identifies you or that could reasonably be used to identify you. Examples of Personal Information include name, address, telephone number, and email address. When this information is combined with any information on health or medical status, we refer to them as “health-related information.” We collect your health-related information only with your consent or if required by law.


Any personal data that you provide or that we collect about you will be stored and retained for a certain time period in accordance with our Corporate Record Retention program, after which it will be archived or deleted. Certain information could be kept for longer periods of times if it is required by law.

You don’t have to register to receive most of the information on our website. However, some of our content is available only to registered users and will require you to set up a profile or provide specific information about yourself in order to fulfill the services you need.

You may contact Curatek to ask questions, discuss your concerns, or report issues. If you communicate with or request information, you may be asked to provide your contact information as well as any health-related and/or personal information that is relevant to your request.
